How to File an FSA Claim

Use this quick guide to submit a claim and track your reimbursement.

  1. Collect documentation — receipt or EOB showing date, provider, amount.
  2. Log in — open the member portal: Employee Benefits Login.
  3. Start a new claim — choose Pay Me or Pay Provider, select expense type and amount.
  4. Attach documentation — upload receipt/EOB; add description and service category.
  5. Submit & track — view status in your dashboard; reimbursement typically processes in a few business days.
